Marriage Prep Intake Form

We’re so glad you’re taking intentional time to prepare for marriage. Our desire is to come alongside you as you build a healthy foundation for your life together.

As part of the Marriage Prep experience, you’ll meet 6–8 times with a trained facilitator couple who will walk alongside you through a premarital workbook and intentional conversations designed to help you prepare well for marriage.

The cost for the Marriage Prep experience is $99 per couple. If finances are tight, please let us know. We are happy to help make this experience possible—we never want cost to be the reason you don’t receive good care and support in this season.

This form will help us get to know you, learn a little about your relationship, and connect you with the right facilitator couple.
