Classroom Care Plan

Does your child have special needs that my need to be addressed in the classroom? The Classroom Care Plan is a form that Grade School Ministries has used to learn about and connect with children who are “differently-abled” so that we can work with their families to prepare a place for them. If you would like to explore finding a place for your child within SAC Children's Ministries, please download the form below.

Do you have a Special Concern for your child?

Part of ensuring that time spent in His Kids is “the best hour of every child’s week” is to be certain that anyone who interacts with your child is aware of his or her special needs or any concerns you may have. The  “Special Concerns form” is a tool that we use to become aware of any way we can better anticipate and prepare for a child with special needs or concerns. We want your child to be comfortable and well cared for here with us. You can download a form at the bottom of this page.  Print it out, fill it in and return completed forms to Cindy Hayden’s mailbox or to any His Kids staff person.
